The U.S. Chemical Safety Board provided an update on its ongoing investigation into the explosion and ensuing fire that occurred at the Hoeganaes facility on May 27 in Gallatin, TN, in which two workers died and a third was gravely injured. It states that preliminary examinations show many violations of safety practices developed by the National Fire Protection Association for combustible metal powder.
